Urgent referrals
For a suspected retinal tear or detachment, endophthalmitis, acute retinal artery occlusion, or sudden vision loss, please call us during office hours at (305) 712-6711 rather than faxing, and ask to speak with our clinical staff.
What to include
- Patient name, date of birth and phone number
- Referring doctor's name, phone and fax
- Working diagnosis or reason for referral, and urgency
- Medical insurance (and the referral/authorization itself, if the plan requires one)
- Any recent imaging you have (OCT, fundus photos, visual fields) and relevant notes
You may direct the referral to Dr. J. Daniel Diaz or Dr. Anne L. Kunkler, or to the practice, and we will schedule with the first available surgeon.
What your patient can expect
We call the patient to schedule, typically within one business day of receiving the referral. The first visit takes one to two hours and includes dilation and imaging; we ask patients to bring a driver. After the visit, a consultation letter with findings and plan is faxed to your office. We are happy to co-manage, and we return patients to you for routine care whenever appropriate.
Conditions we see
Age-related macular degeneration, diabetic retinopathy and diabetic macular edema, retinal vein and artery occlusions, central serous chorioretinopathy, retinal tears and detachments, macular holes and epiretinal membranes, posterior vitreous detachment and lattice degeneration, uveitis, and all vitreoretinal surgery. We manage uveitis that responds to local and steroid therapy; patients with advanced or recurrent disease requiring systemic immunomodulatory therapy are best served by a dedicated uveitis specialist, and we are glad to suggest one. If you are unsure whether a referral is warranted, call us. A quick conversation is often the fastest answer.
Education for referring practices
Our Director of Professional Relations, Dr. Diana Shechtman, OD, FAAO, is a fellow of the American Academy of Optometry and a frequent national lecturer on retinal disease. Optometric practices interested in case discussions or in-office education are welcome to get in touch.
